How the integrated solution works: A continuing care provider receives a patient referral by fax, phone or electronically in the PatientPlacement.com Referral Management System. The admissions team receives an alert and assigns and tracks review tasks for the referral. Once the patient is accepted, the admissions or clinical staff selects the Qtrac option from a drop down box of data formats. The staff can import the data directly into the Qtrac system to use as the basis for an electronic medical record and patient clinical management.
